Late-night discharges and last-minute scheduling are no problem. One call connects you with a live dispatcher — any hour, any day, including weekends and holidays.

Four Qualities.
One Standard of Care.

A fine sapphire is evaluated on four qualities: Color, Clarity, Cut, and Carat. We built our entire operation around what those qualities mean when applied to human care.

Color · Integrity

Honest & Transparent

A sapphire's color is its truest identity — consistent and undeniable in any light. We bring that same consistency to every ride: honest pricing, no hidden fees, no surprises.

Cut · Wisdom

Trained & Prepared

A sapphire's cut is the craftsman's judgment — the decisions that reveal its brilliance. Our drivers bring that same trained, thoughtful judgment to every patient interaction.

Clarity · Clarity

Clear Communication

A gem free of inclusions. A service free of confusion. Pre-trip confirmations, ETA updates, and live dispatch so patients, families, and facilities always know exactly where we are.

Carat · Endurance

Always Available

Carat reflects substance — what a stone carries. We carry what matters most: the trust of patients who depend on us. 24/7, 365 days a year, on holidays, weekends, and late nights.

I travel for dialysis three times a week. Having a driver I trust makes the whole experience so much less stressful. I can't imagine going back to relying on family for every trip.

Why the Black Hills
Needs Us

We didn't start a generic transportation company. We started one specifically for the people and geography of the Black Hills — because no one else was doing it right.

🏔️

Closing the Rural Gap

Patients in Haakon, Jackson, and Oglala Lakota counties face 60–90 minute drives just to reach specialist care in Rapid City — with no reliable transport option. We're changing that. A missed ride shouldn't mean a missed appointment.

Ready for the Region's Peaks

The Black Hills is home to Sturgis Rally, national park tourism, Deadwood, and major events that draw hundreds of thousands of visitors. When the region surges, medical transport needs surge with it. We're built to handle it.

📍

Always Local. Always Here.

A locally-owned operator who answers the phone at 2am is fundamentally different from a national dispatch service. We live and work in the Black Hills. Your ride isn't a ticket in a queue — it's a neighbor helping a neighbor.

A Local Company
Built on Service

Black Hills Sapphire Transportation LLC is a locally owned non-emergency medical transportation company serving the Black Hills Region of South Dakota. We founded this company with one goal: to ensure that distance and mobility challenges never stand between a patient and the care they need.

Whether you're a healthcare facility coordinating patient logistics, a family navigating a loved one's ongoing treatment schedule, or an individual who needs a reliable ride — we are your partner.

Our uniformed drivers, clean vehicles, and commitment to punctuality reflect the same care and precision that defines a fine sapphire: integrity in every facet.
